For parents who are enrolling their child in Years 1 to 6, we ask that they bring their child with them to the enrolment interview. 

Please download the enrolment documents to the right of the page or come into our office to collect an enrolment pack. 

Complete this pack and bring this back to the office together with the following documentation:

• Birth Certificate
  
• Last school report card
  
• Professional assessments/reports from the past 18 months
  
• Proof of residency

Enrolment prerequisites

Please complete the attached enrolment application forms and return to the administration along with the following original documentation.

1. Proof of residency
2. Australian Birth Certificate or 
3. Passport and Birth Certificate from country of birth for students born overseas
4. Visa details for non-Australian resident students. 
   

Tours

Tours of the school are offered as part of the enrolment interview process at Undurba State School.
